基于巢式病例对照研究的省级儿童医院住院患儿 感染性腹泻影响因素分析

Analysis of Influencing Factors for Infectious Diarrhea in Hospitalized Children in Provincial Children’s Hospitals Based on Nested Case-Control Study

摘要: 目的:基于省级儿童医院住院患儿队列开展巢式病例对照研究,探讨住院患儿发生感染性腹泻(Infectious diarrhea,ID)的影响因素。方法:收集 2021 年 6 月至 2023 年 6 月消化内科病区收治的住院患儿临床资料,采用前瞻性与回顾性相结合的方法,依据是否诊断为 ID 分为病例组(n=326)与对照组(n=992)。比较两组各项指标,总结 ID 患儿的临床特征,分析 ID 发生的影响因素,并对病原学诊断结果进行分析,比较细菌性与病毒性 ID 的临床特点。结果:细菌感染 261 例、病毒感染 107 例、混合感染 42 例,主要包括沙门菌肠炎 139 例、细菌性肠道感染 93 例和轮状病毒性肠炎 72 例。所有 ID 患儿均有不同程度腹泻,部分伴有发热、恶心 / 呕吐等症状。患儿年龄集中于 5 岁以下,不同性别年龄分布无统计学差异(χ²=2.096,P=0.351)。ID 发病呈季节性分布,夏秋季高发,尤其 6–8 月。单因素分析显示,两组在卫生习惯、饮食情况和生活条件方面差异有统计学意义(P < 0.05);Logistic 回归分析表明,上述因素均为 ID 发生的独立危险因素(P < 0.05)。病原体分布以细菌为主(80.06%),病毒次之(19.93%),主要病原为沙门菌和轮状病毒。结论:儿童 ID 发病存在季节性高峰,男性略多,病原以沙门菌和轮状病毒为主。不良卫生习惯、饮食及生活条件可增加发病风险,应加强针对性防控。

Abstract

Objective:To conduct a nested case-control study based on the cohort of hospitalized children in provincial children's hospitals, and explore the influencing factors of infectious diarrhea (ID) in hospitalized children.Methods:Clinical data of hospitalized children admitted to the Department of Gastroenterology from June 2021 to June 2023 were collected. Using a combination of prospective and retrospective methods, the children were divided into a case group (n=326) and a control group (n=992) according to whether they were diagnosed with ID. Indicators between the two groups were compared to summarize the clinical characteristics of children with ID, analyze the influencing factors of ID occurrence, and analyze the etiological diagnosis results to compare the clinical characteristics of bacterial and viral ID.Results:There were 261 cases of bacterial infection, 107 cases of viral infection, and 42 cases of mixed infection, mainly including 139 cases of salmonella enteritis, 93 cases of bacterial intestinal infection, and 72 cases of rotavirus enteritis. All children with ID had diarrhea of varying degrees, and some were accompanied by symptoms such as fever, nausea/vomiting. The age of the children was mainly under 5 years old, and there was no statistically significant difference in age distribution between different genders (χ²=2.096, P=0.351). The incidence of ID showed a seasonal distribution, with a high incidence in summer and autumn, especially from June to August. Univariate analysis showed that there were statistically significant differences between the two groups in terms of hygiene habits, dietary status, and living conditions (P<0.05); Logistic regression analysis showed that the above factors were all independent risk factors for ID occurrence (P<0.05). The distribution of pathogens was dominated by bacteria (80.06%), followed by viruses (19.93%), and the main pathogens were Salmonella and rotavirus.Conclusion:The incidence of ID in children has a seasonal peak, with a slightly higher number of males. The main pathogens are Salmonella and rotavirus. Poor hygiene habits, diet, and living conditions can increase the risk of ID, so targeted prevention and control should be strengthened.
